Provide more descriptive warning to users wishing to export to BOADICEA

Description

It's not clear to Phenotips users how much information needs to be put in the pedigree in order to export a file that can be used in BOADICEA. I think there is a very simple answer though.

I'd like to improve the dialogue box to say:

2) BOADICEA requires that all individuals with cancer have their year of birth and age at cancer diagnosis specified or estimated.

3) BOADICEA recommends that all unaffected individuals have their year of birth and year of death, if applicable, specified or estimated. Not doing so may lead to an overestimation of risk.

I'm also wondering what the checkbox at the bottom means? Is it like the "Do not show this message again" boxes often used by Windows?
