Reword the following:
"This patient has been uploaded to this server less than an hour ago as P0000031.Further pushes will attempt to update the patient on the remote server."
"This patient was uploaded to Local555 two days ago. The new ID on Local555 is P0000003"
"If data in the record has changed, push to Local555 again to update the record in both databases."
In general, avoid using "this" because it is not immediately obvious what you are referring to. Specifying the name of the remote instance is preferred. In this case, Local555.
Also, the term "Remote server" is not common language!
I feel like the wording under "Consents" is difficult to understand. I would also change it to "Consent" singular.
The remote server is configured to accept some data only if the patient has consented to collection and/or sharing of such data. The checkboxes below should be checked for consents that the patient has granted. Not all consents are required, but some data might be not uploaded if a corresponding consent is not granted.
Please confirm that consent has been obtained from the patient. Not all categories are required, but may prevent the transfer of some data to Local555
What is the reasoning behind the "I agree to push this data to the server" checkbox?
1. Ambiguous...what server? Should say Local555 or whichever server has been selected
2. Why do they have to check the box? Is clicking "push" not confirmation enough?
3. Why put the check box before field selection and not immediately before the push button.
