Unable to remove collaborator after modifying owner

Description

Opened a patient record and attempted to change the owner.
After successful update, the patient record reflected the new owner, but showed no collaborator even though the former owner was assigned as collaborator.
Opened the Modify permission dialog and attempted to remove the collaborator.
Got an error message stating the the system was unable to update the record even though the update was successful.
